We’d love to meet you and your pet! New patient appointments can be scheduled online or by calling the clinic at 970-663-6046.
Upon arrival at the clinic, we will ask you to fill out a new client form to get to know more about you and your pet and to ensure that your pet’s medical record is complete. If you would like to fill out this form in advance, it can be printed and brought with you to the appointment or emailed to blueskyreminder@blueskyclinic.com prior to your visit.
If your pet is experiencing a medical emergency, please contact a local emergency vet for immediate care.
